The oral frenula – frenulum labii superioris, frenulum labii inferioris, and frenulum linguae – are connective tissue structures within the oral cavity whose clinical significance extends beyond their stabilizing role. When abnormally inserted, they may be associated with orthodontic, myofunctional, speech, and periodontal disturbances, particularly during the mixed dentition period. This review provides a comprehensive analysis of anatomical variations, diagnostic tools, and therapeutic protocols addressing morphological or positional abnormalities of the oral frenula. Well-established classification systems (Placek et al., 1974; Rui et al., 2019; Kotlow, 1999; Marchesan, 2005; Hazelbaker – ATLFF, 1993), functional tests, and standardized photographic/videographic documentation are discussed. Management algorithms for diastema, gingival recession, and ankyloglossia are reviewed, emphasizing the importance of interdisciplinary collaboration involving the orthodontist, speech therapist, surgeon, and periodontist. Special consideration is given to the timing and modality of frenotomy or frenectomy, including laser-assisted techniques.

Oral frenula should not be regarded as mere anatomical remnants but as prognostic biomarkers of potential systemic relevance, requiring an individualized and synchronized interdisciplinary approach to achieve long-term orthodontic and functional stability.

“To doubt is uncomfortable, but to be certain is ridiculous.” – M. de Voltaire

Abstract: One method of creating space in the dental arch is the distalization of the posterior teeth. In the 1990s, the first appliances for anteroposterior tooth movement emerged. In 1992, Hilgers developed the first such appliance, called the Pendulum. He combined it with extraoral force, achieving highly effective distal movement. The aim of this study is to systematize information from the literature regarding dentoalveolar and skeletal changes resulting from maxillary molar distalization using dentoalveolar anchorage in non-growing patients, specifically with certain types of distalization appliances. The focus of the research was to examine the specific effects of the Carriere Motion ii appliance.

Materials and Methods: Using search engines such as PubMed, Scopus, Web of Science, ScienceDirect, and Google Scholar, and the following keywords: Carriere Motion ii, maxillary molar distalization effect, secondary specific movements, anchorage in Carriere Motion ii, inclination of incisors, premolars, and molars, a total of 86 publications were identified. We selected 10 types of distalization appliances that function without skeletal anchorage. We systematized the tooth movements occurring in the posterior segment of the maxillary arch, the side effects in the anterior segment, and examined the presence of any movements in the mandibular arch. Additionally, we investigated whether skeletal changes were present during treatment with these appliances.

Results: The amount of distalization of the posterior teeth was measured in millimeters, along with the mesial movement of the upper premolars, protrusion of the upper incisors, and the angulation of the aforementioned teeth. Mandibular teeth movement associated with the use of the Carriere Motion ii appliance were also evaluated. Skeletal changes in the maxillary arch and the duration required to achieve the therapeutic effect were assessed.

Keywords: Carriere Motion ii, distalization effect of maxillary molars, secondary specific movements, anchorage in Carriere Motion ii, inclination of incisors, premolars, and molars.

The involvement of more than one type of specialist in the treatment of adult patients is often necessary to ensure optimal aesthetics and the longest possible preservation of physiological function. The orthodontist’s role primarily relates to preparing the patient for future implant placement and/or prosthetic rehabilitation by creating adequate space and eliminating etiological factors for periodontal deterioration by aligning the teeth, correcting the dental arch, and normalizing occlusal relationships where needed.

There is a scarcity of studies dedicated to the estimation of dental age (da) among children in the local population in Bulgaria. This topic holds considerable importance for clinical orthodontic practice, particularly with regard to early diagnosis and effective treatment planning. Methods used to assess dental age offer high reliability and practical applicability in clinical settings. The Demirjian and Willems methods are among the most widely recognised and used for this purpose. While both demonstrate significant utility, each presents distinct strengths and limitations in the context of dental age estimation. Numerous studies indicate a strong temporal correlation between specific morphological indicators of skeletal maturity marking the onset of puberty and the degree of tooth germ calcification. This correlation facilitates a relatively straightforward estimation of an individual’s growth potential based on dental age. The aim of this study is to critically evaluate and compare the Demirjian and Willems methods using data obtained from 25 Bulgarian children aged 7 to 12 years. In the literature, dental age assessment methods are generally divided into two categories: those suitable from birth to puberty and those applicable from puberty up to 20 years of age. Both the Demirjian and Willems methods are considered appropriate and reliable for the age group examined in this study. Keywords: assessment methods, critical analysis, dental age, children, orthodontics, diagnostics.

Modern orthodontics takes into account and analyzes not only changes in the tooth position and occlusion, but also changes in the soft tissues of the face. To accurately assess these changes during orthodontic treatment, models obtained from 3D facial scans are used. Facial scanners and CBCT examinations provide the ability to capture and assess three-dimensional changes in the soft tissues. This study describes the difference after orthodontic treatment in automatically generated 3D models of the face using CBCT examination. The aim is to localize the areas of change in facial structures during patient treatment. Two patients with transverse maxillary deficiency were selected – one in mixed dentition and the other in permanent dentition. The deficiency is associated with unilateral (in mixed dentition) and bilateral (in permanent dentition) distal crossbite. The patients were treated with a metal-printed appliance for tooth-borne rapid maxillary expansion. 3D bone and facial images were obtained from a CBCT examination performed in the diagnostic phase (T1) and at the end of the retention phase and immediate removal of the appliance (T2). The period between T1 and T2 was 4–5 months. Bone changes in the transverse direction are assessed and superimposition of bone and soft tissue structures is performed. An increase in the width of the maxillary base, hard palate, alveolar bone dimensions, and occlusal width is achieved. No changes in the alveolar width of the mandible was observed, but clinically, its spontaneous rotation and displacement is observed after deblocking the occlusion. The most significant change in the soft tissues is found in the nasal width, the subnasal area, the upper lip, and the area of the zygomaticomaxillary suture. Modern technologies offer various systems (software) for facial imaging. The data obtained outlines the areas of change that would be target fields when searching for discrepancies using facial imaging.

Over the past two decades, modern dental medicine has undergone a technological revolution driven by advances in digital planning, computer-aided design and manufacturing (Cad/Cam), and artificial intelligence. These innovations have significantly changed diagnostics and treatment planning. This shift has moved the focus from fixed appliances to invisible, removable devices called aligners.
