Orthodontic treatment is considered successful not only when the desired results are achieved but also when long-term stability is ensured. due to increasing aesthetic demands from patients, long-term fixed retention is becoming more prevalent, sometimes extending to a lifetime. this necessitates the use of protocols that enhance the adhesion strength of fixed retainers to the tooth surface. one potential method for improving this adhesion is the pre- treatment of the enamel surface with al2o3 through intraoral sandblasting.
objective the aim of this study is to compare the failure rate associated with the debonding of fixed retainers in patients with treated and untreated enamel surfaces using the aquaCare* device.
results the results indicate a significantly lower incidence of retainer debonding in the aquaCare – treated group (14.00%, n = 7) compared to the untreated enamel group (34.00%, n = 16), with a z-score of -2.31 and a p-value of 0.02. among patients with debonded teeth, 85.70% of those treated with aquaCare had only one such tooth, while 14.30% had two debonded teeth. in contrast, within the untreated enamel group, 62.50% had one debonded tooth, 31.25% had two debonded teeth, and 6.25% had three debonded teeth. overall, the proportion of patients with more than one debonded tooth was 37.5% in the untreated group compared to 14.3% in the aquaCare-treated group.

Conclusion: the results of our study demonstrate a significantly lower incidence of retainer failure in the aquaCare-treated group compared to the untreated group. in both groups, retainer failure was more frequently observed in male patients. the majority of patients in both groups experienced the detachment of a single tooth; however, the proportion of patients with multiple debonded teeth was higher in the untreated enamel group. the distribution of debonded teeth showed that lateral incisors were most commonly affected, followed by canines and central incisors.

Key words: fixed retainer, debonding, sandblasting, adhesion.

Introduction: patient feedback is a critical metric for assessing the quality of healthcare services and the engagement level of the dentists. patient satisfaction plays a significant role in adherence to orthodontic treatment, which is usually long lasting and demanding. motivated and cooperative patients are more likely to follow the proposed treatment plan, attend appointments at time, and achieve better treatment outcomes, leading to mutual satisfaction for both patient and clinician.
aim: the aim of this study is to identify the factors influencing patient satisfaction with orthodontic treatment at the faculty of dental medicine (fdm) in mu-sofia. as well to explore the reasons patients seek orthodontic care, the relationship between initial dental deformation and satisfaction levels, and the challenges faced during treatment.
materials and methods: the survey included 52 patients undergoing fixed appliance orthodontic treatment by postgraduate residents in orthodontics at the department of orthodontics, fdm-sofia, between 2021 and 2024. each participant completed a written questionnaire with 12 written questions.
results: participants were 38% male and 62% female. the results from both sexes revealed high levels of satisfaction in the treatment outcomes and the quality of care provided at fdm-sofia.
Conclusion: the survey demonstrated that patients undergoing orthodontic treatment at fdm-sofia are highly satisfied with their results, and the challenges encountered during treatment were minimal.

The orthodontist in his practice finds himself in the role of manager, marketing and financial director, host, hr specialist and even a trainer for his team. for this new role, he is not always sufficiently prepared and only his intuitiveness and education can help him move in the right direction for the development of his business. innovation systems have a key role in the development of clinical orthodontic practices, reducing the need for human intervention, saving time and minimizing errors.

The article presents the most modern technological and innovative methods that are the future of orthodontic practice: direct printing of aligners in the office; robotic bending of arches; use of ai in analyzing 2d (x-ray) and 3d (CbCt) images.

Digitalized orthodontic practice would provide patients with more comfortable, personalized and related to new experiences treatment and care. this encourages orthodontists to implement leading innovations to meet the growing demands in the field.

Craniofacial development is influenced by epigenetic, functional, and locally acting factors and forces. these disturbances are subject to investigation and management by various healthcare professionals, necessitating familiarity with contemporary diagnostic methodologies employed across specialties. mouth breathing, one of the most common detrimental oral habits in children, as results from obstructions in the upper airway. orthodontists are typically the first to identify these issues and refer patients for consultation with otolaryngologists or pediatricians. therefore, orthodontists require a comprehensive understanding of the physiological and pathological changes occurring in the lymphatic oral structures during various stages of development. radiographic diagnostic tools employed by ent specialists, such as CbCt (Cone beam Computed tomography), often overlap with those used in orthodontics. Consequently, interdisciplinary treatment teams must establish a unified approach to treatment planning, leveraging shared knowledge.

This literature review aims to summarize recent advancements in innovative techniques for detecting upper airway obstructions, which are essential for orthodontists to guide their patients effectively. additionally, it briefly outlines the connection between these techniques and orthodontic diagnostics.

An important step forward in dental medicine is the integrated approach in the treatment of patients with complex orthodontic and medical needs. orthodontic treatment, in support of periodontology, improves periodontal health and corrects abnormal gingival and bone topography. the orthodontist directs the movement of the teeth and redistributes space, facilitating future prosthetic rehabilitation. they also collaborate with surgery in cases of congenital anomalies, discrepancies in jaw sizes after growth completion, severe asymmetric growth, and anatomical limitations. an important role is also played in the treatment of osa (obstructive sleep apnea) by considering the anatomical features leading to this condition. a key aspect is the joint creation of a treatment plan that ensures the achievement of long-term oral health.
